Instructions for Group Leaders

Thank you for your interest in Cognosco et Credo’s New Families Fund. Please note the following instructions:

This Recommendation Form must be completed by the leader of a sponsoring organization before a prospective family may apply for financial assistance through the New Families Fund.
Verify that any prospective applicant meets the Eligibility Requirements listed below before submitting a Recommendation Form on their behalf.
Notify the prospective family when you have submitted the Recommendation Form. Applications will be sent directly to eligible families once your form has been reviewed.
Recommendation Forms must be completed by a leader, director, or officer of the sponsoring organization.
Eligible sponsoring organizations include (but are not limited to) homeschool support groups or co-ops in which the prospective family is enrolled for the current year.
Leaders may submit a Recommendation Form on behalf of one prospective family at a time.
The contents of Recommendation Forms are strictly confidential and will not be shared with Applicants.

Applicant Eligibility

Families must be Oklahoma residents living within the Northeastern Oklahoma area.
Families must have at least one school-age child (4 to 18 years) who meets all of the following criteria. This student:
• Must reside in the primary or joint custody of the applicant; and
• Must receive a full-time, parent-directed, privately-funded education based in the same home.
Families must apply for assistance prior to, or during, their first academic year of homeschooling.
